Vocal cord granuloma steroid injection

• Recently, there have been several studies that have shown that steroid injection directly into vocal cord nodule can resolve or reduce the nodule resulting in improved vocal quality within weeks. Such local injection technique has mainly been performed in the treatment of spasmodic dysphonia (botox injection), vocal cord granulomas , and vocal cord paralysis . Watch a video how a "local injection" to the vocal cord can be performed (video shows injection of vocal cord granuloma rather than nodule, but overall approach is identical).

When both vocal folds are paralyzed, they are usually immobilized close to the midline. Thus, closure is usually adequate, permitting acceptable voice and swallowing. However, breathing is usually very difficult. This difficulty increases with effort, and people with this condition may find that they are severely restricted in how far they can walk, or in how heavy a load they can carry. Often there is a wheezy noise when breathing, which can cause this condition to be mistaken for asthma. Asthma that fails to improve with appropriate medicines and inhalers requires that a physician examine the larynx and vocal folds.

Aspiration pneumonia is typically caused by aspiration of bacteria from the oral cavity into the lungs, and does not result in the formation of granulomas. However, granulomas may form when food particles or other particulate substances like pill fragments are aspirated into the lungs. Patients typically aspirate food because they have esophageal, gastric or neurologic problems. Intake of drugs that depress neurologic function may also lead to aspiration. The resultant granulomas are typically found around the airways (bronchioles) and are often accompanied by foreign-body-type multinucleated giant cells, acute inflammation or organizing pneumonia. The finding of food particles in lung biopsies is diagnostic. [12]
